Universal access to sanitation a far cry

Universal access to sanitation a far cry

MAR 12 -
No matter how many Open Defecation Areas (ODFs) Nepal declared in recent years, the country is likely to miss the Millennium Development Goals and the ambitious goal to ensure universal sanitation and safe drinking water by 2017.

After the government in 2002 announced to declare all the VDCs open defecation free by 2017, only 300 of the total 3,915 units have been declared ODF in 75 districts. Moreover, how the declared ODF areas are faring has hitherto sunk into obscurity owing to lax monitoring and lack of follow-up by the government. This, in addition, has raised a serious question about sustainability of the much-talked-about campaign.

The Sanitation and Hygiene Master Plan has laid emphasis on the post-ODF situation stressing the involvement of local communities. The plan, which incorporates ODF as a major component, is yet to be implemented as a majority of the districts have not made any strategy so far. As of now, 40 districts have come up with a strategic plan, believed to help speed up the campaign.

Open defecation, rampant particularly in the Tarai, has badly contaminated water sources, raising serious public health risks, according to a UNDP progress report on the Millennium Development Goals. Some 13,000 children below five die of diarrhoeal diseases annually in Nepal, which can be partly attributed to poor hygiene and sanitation, according to the report.

Another report states that every day around 15 million Nepalis openly defecate mainly due to the lack of toilet. Hence, the ODF campaign can ensure a toilet in every household and help them practise the behaviour all their life, only if it is implemented in a sustainable manner with community participation, say experts.

“ODF campaign still remains a challenge in the central Tarai. People are not motivated to adopt healthy living behaviours,” said Nanda Bahadur Khanal, senior divisional engineer at the Ministry of Physical Planning and Works. “Also, lack of political commitment to own up the campaign makes it hard to push the drive in many places.”

For instance, Khanal said that the ODF campaign in Rautahat shuddered to a halt as there is no Local Development Officer. He said workforce crunch at the grassroots level had also affected the programme and monitoring process.

Legal Advisor to the Solid Waste Management and Resource Mobilisation Centre Dipendra Oli sees three challenges surrounding the ODF campaign. “First is the economic challenge. The other is the utilisation of built resources and the last sustainability of the programme.”

Oli stressed that the programme should make a slow and steady progress. “A decade ago, the then GTZ had given us necessary resources and technical support in managing garbage in the Kathmandu valley. After they handed it over to us, we headed nowhere. These kinds of failure are plenty in Nepal. Therefore, the government has to bring the communities on board practically for its sustainable development.”

“What will a poor family in hilly region do if a toilet made by an organisation is washed away by a flood? Therefore, along with the sanitation programme, their economic sustainability should also be taken into consideration and infrastructure should be built according to the demand of the community and not through high-flown plans.” While a WaterAid report estimates that Nepal needs a whopping annual investment of Rs 7.5 billion to meet the sanitation and drinking water goal, experts say that not money, but working sincerely towards the goal, would put Nepal to the test.

Ashutosh Tiwari, country representative of WaterAid Nepal, said the government has spent around one-and-a-half years harmonising organisations working in the field of sanitation. “Coordination among the stakeholders has made our work much easier. Now the government has to wholeheartedly work to achieve the sanitation target.” He said the government should have a strong monitoring mechanism to create positive effects of the campaign in the society and also understand the status after ODFs are declared.

“For this, the government plan to hand over the responsibility of planning, implementation, monitoring and supervision of sanitation and hygiene activities to the VDC-level Village Water, Hygiene and Sanitation Coordination Committee is really commendable,” Tiwari said. Nepal is going to host the fifth South Asian Conference on Sanitation in 2013. “To show achievements gained so far in this conference, Nepal needs to accelerate work and it is the right time for the government to roll up its sleeves,” Tiwari said.

84 open spaces in Valley for emergency accommodation

Ankit Adhikari/KATHMANDU, DEC 26 -
If a major quake strikes the Capital city, more than one million people are likely to be homeless. One of the biggest challenges in the Kathmandu Valley will be finding spaces to accommodate the homeless. The government of Nepal and UN agencies have attempted to address this problem.

In a joint study conducted by the Ministry of Home Affairs (MoHA) and the International Organisation for Migration (IOM), 84 open spaces have been identified in Kathmandu that could be used for humanitarian support in the event of a crisis. In those 84 spaces, the report says, 792,698 Internal Displaced Persons (IDPs) can be accommodated, but in a very crowded condition, which is likely to increase other humanitarian problems like water crisis, sanitation, and food.

The project has categorised large, medium and small areas in Kathmandu that have the potential to be used as accommodation centres or for other purposes for the IDPs. According to the report, an estimated population of 3.5 million is currently living within the confines of the Valley. Considering the high number of sub-standard constructions, a major earthquake measuring 9 on the Richter scale would destroy about 60 percent of these buildings. The report recommends that schools be used as reception centres where IDPs may assemble for registration, following which their placement procedure could begin. For this, the government has to identify retrofitted schools in the Valley as soon as possible, the report suggests, adding that 3,150,000 square meters worth of open area would be required to provide 900,000 people with covered shelters. The report offers a list of potential sites by identifying them at three different levels—large, medium and small. Large camps have been defined as those that could accommodate 30,000 to 90,000 people. The study identified two large blocks of land (875,043 square meters) at Tribhuvan University in Kirtipur and the National Agricultural Research Centre in Lalitpur (305,470 square meters) that could function as camps falling under the “large” category.

According to the report, these areas could hold up to 118,051 people together and be suitable for long-term accommodation, with the provision of some of the site services within the camps. Medium-sized camps have been defined as those that could accommodate 20,000 to 45,000 people in a covered area (3.5 square meters per person). The report suggests six medium camps with the capability of holding up to 286,908 people. The planning for this is based on only providing covered living accommodation, with other camp facilities outside the site. The Ratnapark/Tundikhel area (155,400 square meters), the airport and Golf Club site (253,327 square meters), the southern end of the runway of the International Airport (146,575 square meters), the Oxygenation Park on the banks of Bagmati River opposite Tribhuvan University (146,604 square meters), Gokarna Golf Club (178,212 square meters) and Birendra Sainik School in Bhaktapur (134,564 square meters) have been proposed as potential medium camps. The small camps, the report defines, are 75 places that could accommodate accommodate approximately 387,739 people—3.5 square meters per person. Among the possibilities are Satdobato, Balkumari, Jawalakhel, Pulchowk, Pulchowk Engineering College, Lagankhel, Tribhuvan University, Bagbazaar, Naxal, greater Pashupati, Sinamangal, Lainchaur, Teku, Chabahil, Balaju, Sallaghari-Tinkune-Bhaktapur, among various other schools and colleges.

“The density of the camps may pose significant challenges to water, sanitation, and hygiene (WASH), food distribution and security,” the report reads. “Water emerged as the most critical of the planning factors. In the event of an earthquake, it is expected that 95 percent of water pipes could be destroyed and pumping capacity reduced by half. Using Sphere Standards of 15 litres per person per day, 13.5 million litres per day would be required for the IDPs. Additional water would be required for the rest of the population, which would be equivalent to 52.5 million litres per day (based on a population of 3.5 million).” Stating that a UNICEF project has already constructed five deep tube-wells in Lalitpur district in preparation for a potential disaster, the study recommends that additional such tube-wells be built after receiving appropriate technical advice on whether or not the wells can survive an earthquake. Going according to the WASH requirements in the event of a crisis, the report suggests one latrine be maintained for every 10 households or 50 individuals.  Similarly, the report recommends the maintenance of one hand pump and bathing space each for every 20 households or 100 individuals and a garbage pit for every 40 households or 200 individuals.

KATHMANDU, DEC 02 -

The Cabinet, late on Friday, declared the western area of Lokanthali, the Manahara bridge area and slums adjoining the bridge ‘bird-flu emergency areas.’

The move was, however, in sharp contrast to the stand taken earlier in the day by the Ministry of Agriculture and Cooperatives, which had declared Kathmandu a ‘bird flu-free zone.’

According to government spokesman and Deputy Prime Minister Narayan Kaji Shrestha, a meeting of the Cabinet decided to take ‘extreme’ measures to prevent the spread of the flu to other parts of the Valley and also keep the area under surveillance.

However, senior veterinarian and the focal person of the Rapid Response Team (RRT) deployed in the affected area, said the government should have taken this decision much earlier as they have already intervened in the areas and done the needful.

In a press conference on Friday, a senior official from the Agriculture ministry said the Manohara area is free from the H5N1 virus, which also infects humans. A senior veterinary doctor, Dr Narayan Prasad Ghimire, had also said at the conference, “Kathmanduites need not worry about the flu as it is under control. The site of the outbreak will be kept under surveillance for 42 days and till then people there will not be allowed to rear fowls.”

An official involved in monitoring the outbreak was, however, quick to say that the declaration was made in haste. The official, who did not want to be named for fear of retribution, said the site should have been kept under observation for at least another week.

“The RRT members who slaughtered the birds on Tuesday have to be under observation for a week as they have high chances of developing bird flu symptoms,” said a veterinary doctor, requesting anonymity. “It has been just four days. Also, the pond close to the outbreak site is the only one in the entire area where all the birds go to swim. Chances are still high that the birds will be infected again.”

Despite claims from government officials that it has quarantined the area, many fowls are still seen in the nearby areas. Experts say the faeces of the fowls are a major source of infection.

Officials are, however, trying to play down the risk. Dr Nar Bahadur Rajwar, Director general at the Department of Livestock Services, said people can consume chicken and can also go on with their poultry business without fear. On Thursday, the government formed a seven-member RRT that will keep people in the outbreak area under surveillance for 10 days.

“We have formed an experts’ team that will be observing the health of the residents in the area. We have also requested all the people there to visit the nearby Lokanthali sub-health post if they develop any Influenza like symptoms (ILS),” Dr GD Thakur, the chairman of the RRT and Director at the Epidemiology and Diseases Control Department (EDCD), said. The general symptoms of the disease include fever of more than 100.4 F, cough, sore throat, muscle ache and difficulty in breathing.

The RRT team on Friday visited the outbreak site where no one reported the symptoms, said Kumar Dahal, the Public Health Inspector at the Avian Influenza Control Project.

KATHMANDU, DEC 01 -
People living in and around the bird flu outbreak site in Madhyapur Thimi Municipality in Bhaktapur district on Thursday said they are being stigmatised and that their children have been harassed in schools.

Twenty-five private schools at Lokanthali and Gatthaghar have been closed for three days—Friday to Monday—due to the outbreak, affecting hundreds of students. “Whenever I go to Koteshwor, people who know me avoid me,” said a seemingly frustrated Deepesh Ghising. The locals there lashed out at government officials and the media. Government officials who visited the site to make people aware of the precautionary measures could not communicate properly with the irate locals. “The officials came and slaughtered the fowls. And now पेओप्ले have started avoiding our children too,” an irate resident said, adding that her daughter was sent back from school.

“We have already lost our source of livelihood to the flu. And our children, for whom we have been doing all this, are being stigmatised,” the local woman added. District secretary of Private and Boarding Schools Organisation (PABSON), Bhaktapur, Shree Krishna KC said many students from the outbreak site were facing problems in school as others tried to avoid them—forcing school authorities to close down 25 schools lying in the periphery of the site. KC said PABSON came up with the decision to shut the schools after the government did not take proper precautionary measures. “Also, parents pressured us to shut the schools to protect the children,” KC said.

Meanwhile, people at the outbreak site said they were not satisfied with the compensation they got for their fowls from the government. The owners of the birds that were slughtered got a total amount of Rs 39,862 as compensation. Officials had killed 308 chickens and ducks and destroyed 14 eggs and around 2 kg of poultry feed. Officials said no one in the area has developed symptoms of bird flu, while those who have been complaining of the symptoms have already been given ‘symptomatic treatment’.

“The symptoms are different in each case, while the flu threat is minimal in the area. Chemicals have been spread in every nook and corner of the shanties,” Kumar Dahal, the Public Health Inspector and an official of the Avian Influenza Control Project (AICP) said. He added that people are prone to the virus for a week after the outbreak. “However, almost 20 days have passed now and we don’t have much to fear,” Dahal said.
