Bajura villagers in grip of diarrhoea


BAJURA, AUG 13 -
Many people have taken ill due to diarrhoea that spread in Badhu VDC in the district for the past week.
Gamba Bahadur Sunar, a retired policeman, said many villagers have taken ill, some seriously, due to the disease at Wards No 2, 3, 4 and 5. He said that Chinkala Rokaya, Krishna Datta Pandey and Shanti BK, who were suffering from the disease, were taken to Ratapani Health Post in the neighbouring Mugu district.
The District Public Health Office (DPHO) said it has not received any formal information about the disease yet. Officiating DPHO Chief Dr Hridaya Bibhu Ghimire said his office would do the needful after receiving information about the epidemic.
Meanwhile, the DPHO in Doti claimed that diarrhoea is under control in the district. Mahendradhoj Adhikari, an official at the DPHO, said though many people died of diarrhoea in the past, the number of deaths caused by the disease was on the decline lately. Tek Bahadur Ojha, inspection officer at the local branch of Epidemiology and Diseases Control Division (EDCD), said the number of diarrhoea patients visiting health centres had decreased lately. Many organisations such as Nepal Police, Nepal Army and Nepal Red Cross Society have launched a campaign against the disease in the district.

Diarrhoea outbreak claims one more



DOTI, JUL 31 -
A six-year-old girl, Ganga Nagari, died of diarrhoea while undergoing treatment at a clinic in Khatiada VDC of Doti on Monday. Only last Saturday, 12-year-old Padma Kunwar from Basudevi VDC had succumbed to the disease that has now spread to many parts of the district.
The District Public Health Office has claimed that the diarrhoeal outbreak in Doti is in check, despite the disease claiming nine lives since mid-June and placed hundreds more in the hospital.
However, Gobinda Joshi of the Doti chapter of the Nepal Bar Association said that the government and other concerned authorities have not taken any concrete measures to control the disease. Joshi warned that more people could lose their lives if authorities continued to ignore the situation.

हैजाको पुष्टि

डोटी, श्रावण १ -
जेठ अन्तिम सातादेखि डोटीको बगलेखमा फैलेको झाडापखालाका बिरामीमा हैजाका जीवाणु रहेको पुष्टि भएको छ ।

जिल्ला स्वास्थ्य कार्यालय डोटी र सेन्टर फर मनीकुलर डाइनामिक्स काठमाडांैले पठाएको झाडा- पखालाका बिरामीको दिसा र उनीहरूले प्रयोग गरिरहेको पानी राष्ट्रिय महामारी तथा रोग नियन्त्रण महाशाखा काठमाडांैले परीक्षण गरी हैजाको जीवाणु रहेको पुष्टि गरेको हो । रोग नियन्त्रण महाशाखाले परीक्षणबाट 'भिब्रयो कोलेरा ओआई ओगावा' जीवाणु भेटिएको प्रतिवेदन दिएको जिल्ला स्वास्थ्य कार्यालयका अधिकृत टेकराज जोशीले बताए ।

जोशीका अनुसार परीक्षणका लागि झाडापखालाका ८ बिरामीको दिसा र पानीका तीन वटा नमुना पठाइएको थियो । नमुनामध्ये ५ जना झाडापखालाका बिरामीको दिसामा जीवाणु पाइएको पुष्टि भएको हो । डोटीमा पखालाबाट जेठ अन्तिम सातायता १ महिना अवधिमा ५ जनाको मृत्यु भइसकेको छ । बगलेखको ६ र ७ वडामा जिल्ला स्वास्थ्य कार्यालयले शिविर सञ्चालन गरी झाडापखाला नियन्त्रणमा ल्याएको बताएको छ । उक्त गाउँका ३ जनाको झाडापखालाबाट मृत्यु भएको हो भने ५ जनामा हैजाका जीवाणु रहेको पुष्टि भएको हो । २ वर्षअघि समयमै ध्यान नदिँदा डोटीमा पखालाबाट १२ जनाको मृत्यु भएको थियो ।

राजधानीमा हैजा

वषर्ायामसँगै राजधानीमा हैजा देखिएको छ । ताहाचल, कालिमाटी र टेकु क्षेत्रमा बसोबास गर्नेमा हैजा देखिएको हो । विज्ञहरूका अनुसार उपत्यकामा निरन्तर हैजा देखिनुको मुख्य कारण खानेपानी र ढलको पाइपलाइन आपसमा मिसिनु लगायत हो ।

समयमै उपचार गरे मृत्युदर करिब शून्य रहने गरेको हैजा/झाडापखालाबाट जाजरकोट, रुकुम, लगायतको पहाडी क्षेत्रमा २०६६ सालमा सरकारी तथ्यांकअनुसार ३ सय ५४ जनाभन्दा बढीको मृत्यु भएको थियो । 'हाम्रो अस्पतालमा झाडापखालाको उपचारका लागि भर्ना दुई महिला र एक पुरुषमा प्रयोगशाला प्रमाणित हैजा देखिएको छ,' शुक्रराज ट्रोपिकल तथा सरुवा रोग अस्पताल, टेकुका डा.राजेश साहले बिहीबार भने, 'यो सँगै उपत्यकामा यो वर्षसमेत हैजाको प्रकोप सुरु भएको छ ।' डा.साहका अनुसार हैजा देखिएका सबैले धाराको पानी खाने गरेको बताएका छन् । हाल टेकु अस्पतालमा दिनहुँ १५ जनासम्म झाडापखालाका रोगी आउने गरेको डा. साहले बताए ।

विश्व स्वास्थ्य संगठन -डब्लूएचओ) का अनुसार हैजाको उपचार नगरे यसले केही घन्टामै संक्रमितलाई मार्न सक्छ । डब्लूएचओको अनुमानअनुसार विश्वमा बर्सेनि ३० देखि ५० लाखमा हैजाको संक्रमण देखिन्छ भने एक लाखदेखि एक लाख २० हजार व्यक्तिको मृत्यु हैजाकै कारण हुन्छ । रोग प्रतिरोधक क्षमताको कमी भएकाहरू जस्तै- कुपोषित बच्चाहरू र एचआईभी संक्रमितहरू हैजाको संक्रमणले मृत्यु हुने जोखिममा बढी हुन्छन् ।

'तहाचल, टेकु, कालिमाटीको तीनवटा घटनामा 'जिरो वन ओगावा' प्रजातिको हैजा देखिएको हो,' अस्पताल प्रयोगशालाका सूक्ष्मजीव विशेषज्ञ ज्योति आचार्यले भनिन्, 'यो मौसमको पहिलो हैजाको घटना हो ।'

उपत्यकामा प्रयोगशाला प्रमाणित हैजाको निरन्तरता १५ औं वर्षमा पुगेको इपिडिमियोलजिस्ट डा. मानसकुमार बनर्जीले बताए ।  'उपत्यकामा हरेक वर्ष हैजा देखिन्छ, कम्तीमा हामीले प्रयोगशाला प्रमाणित नमुनाहरू संकलन गर्न थालेको करिब डेढ दशकदेखि त बर्सेनि देखिन्छ,' डा. बनर्जीले भने, 'हाम्रो अनुमानअनुसार डेढ दशकअघि पनि निरन्तर रूपमा हैजा देखिएको होला ।' 'राजधानीमा निरन्तर हैजा देखिनुको मुख्य कारण भने खानेपानी र ढल आपसमा मिसिनु हो,' उनले भने, 'सरसफाइ, सुरक्षित खानेपानी र खानपानले हैजाबाट बच्न सकिन्छ ।' उपत्यकामा वितरित खानेपानी उपभोक्ताको धारासम्म आइपुग्दा खानका लागि पूर्ण सुरक्षित रहेको ग्यारेन्टी काठमाडौं उपत्यका खानेपानी लिमिटेड -केयूकेएल) गर्ने स्थितिमा छैन । 'हाम्रो रिजर्भवायरमा प्रशोधित पानी डब्लूएचओ गुणस्तरको हुन्छ तर वितरण प्रणालीको सञ्चालन पुरानो रहेको र खानेपानी एवं ढलको पाइपलाइन जमिनमुनि सँगसँगै गएकाले यस्ता फुटेको पाइपलाइनमा के गर्ने,' केयूकेएलका प्रवक्ता सुरेश आचार्यले भने, 'यो मौसममा हामी खानेपानीलाई उमालेर खानु राम्रो हो भन्ने गर्छौं ।'

पानी गुणस्तर शाखा कीर्तिपुरका प्रमुख ज्ञानेन्द्रबहादुर कार्कीले अन्य समयमा वितरण गरिने खानेपानीमा शून्य दशमलव पाँच पीपीएम क्लोरिन हालिने गरिएकोमा हाल झाडापखालाको मौसममा एक पीपीएमसम्म क्लोरिन हालिने गरिएको बताए । हैजा आन्द्राको संक्रमण हो जो भिबि्रयो कलेरी नाउँ गरेको जीवाणुले गर्दा हुन्छ । दूषित पानी र खानाले फैलिने यो रोग आन्द्रामा लाग्छ । यो रोगको जीवाणुले आन्द्राको आन्तरिक पत्रमा आक्रमण गर्छ जसले हैजाको लक्षण देखिन्छ । यसले कोशिकाबाट बढी मात्रामा पानी निस्किन थाल्छ र चौलानी जस्तै दिसा हुन थाल्छ । 

हैजाको संक्रमण भएको २४ देखि ७२ घन्टाभित्र लक्षण देखिन्छ भने प्रथम दिसा चौलानी जस्तै भएको क्रम दुई पटक दोहोरिएपछि तुरुन्त अस्पताल पुर्‍याउनुपर्ने डा. बनर्जीले बताए । लक्षण देखिएको ३ देखि ६ घन्टामा यसले घातक रूप लिन्छ । यो जीवाणुको विषाक्तता धेरै हुन्छ भने यसले हरेक आयुवर्गमा फैलिएर महामारीको रूप लिन सक्छ ।

Capital city at risk of waterborne diseases

With the much-awaited monsoon rains hitting the country, the threat of waterborne diseases looms large in the Kathmandu Valley. More people have started thronging the only government hospital for tropical and infectious disease—Sukraraj Tropical and Infectious Disease Hospital (STIDH)—for treatment.

Waterborne diseases like cholera, typhoid and gastroenteritis break out due to the consumption of polluted water and contaminated food items in the monsoon. “Although the patients have just started coming, we are fully prepared to deal with the cases of waterborne diseases,” said Dr Inda Prasad Prajapati, director of Sukraraj Hospital.

Physician Dr Shanker Bahadur Shrestha said people have to be more careful and start practicing personal hygiene seriously to keep communicable diseases at bay. 

Diarrhoea, cholera, hepatitis A and B, typhoid fever, malaria and also dengue remain a threat with the monsoon arrival in the Kathmandu Valley, said Dr Shrestha. He all should consume clean water, wash hands with soap and water frequently, maintain personal cleanliness and take fresh foods, among other things. 

“The typical symptom of diarrhoea is watery stool. It is dysentery if the stool is mucus and the patient suffers from cramping and fever,” said Dr Shrestha. “Diarrhoea could be dangerous if not treated on time.” 

Similarly, Dr Shrestha said typhoid fever also remains a threat to Kathmandu denizens. Typhoid is a bacterial disease caused by Salmonella typhi. “People get infected after eating contaminated food or drinking beverages,” Dr Shrestha said. “The only way to mitigate the typhoid risk is paying attention to food and drinking habit and referring the people showing those symptoms to the nearby hospital.”

The symptoms of typhoid include high fever, malaise, headache, constipation, and diarrhoea, rose-coloured spots on the chest and enlarged spleen and liver. 

Dr Arjun Raj Panta, pediatrician at STIDH, said every year the hospital detects cholera, which occurs mainly when drinking water contaminated by monsoon rains reaches the pipeline of human beings, posing high risk of cholera infection. He said if symptoms such as diarrhoea and vomiting are seen, patients should be immediately taken to hospital and if the infected are children and elderly, they have a higher risk of infection, which could be fatal. 

“As the roads are covered in dust, there is high chance of cholera outbreak,” said Dr Panta. “People can reduce the chance of contracting cholera by maintaining cleanliness, drinking only treated water or other safe fluids and eating cleaned and well-cooked food.” 

Last year, the District Public Health Office, Kathmandu, recorded around 7,500 cases of typhoid and 12 cases of cholera in the Capital. 

The World Health Organization says Japanese encephalitis is a leading cause of viral encephalitis in Asia with 30,000-50,000 clinical cases reported annually. In 1996, according to the WHO, Nepal saw 697 cases with 118 deaths suspected by Japanese encephalitis and in 1995 the total number of JE cases reported was 772 with 126 deaths.
